Scholarship Application Form – Teacher Workshop

Please provide a reference who is willing to fill out an online recommendation. The recommendation should be from someone who knows your musical background and/or teaching (mentor, previous teacher, colleague). When you submit your scholarship application, you will receive an email with a link to the scholarship recommendation form. It is your responsibility to email that link to your reference. Your scholarship application is complete when your reference has filled out the teacher recommendation form.

CSI Scholarship Committee will hold all information in confidence. Please provide a written statement below explaining why a scholarship is necessary to attend. Please include any unusual expenses or circumstances you would like taken into consideration.

Please mail the first two pages of your current year Form 1040 directly to: Colorado Suzuki Institute, 3440 Youngfield St. #111 Wheat Ridge, CO 80033 OR scan and email to execdirector@coloradosuzuki.org. (If possible, please black out social security numbers before submitting. These documents well be destroyed as soon as the application is processed.)

I have read the guidelines on the enclosed form and if a scholarship is granted, I agree to provide 3 hours of service to the Colorado Suzuki Institute, to be determined upon arrival and to attend a meeting for scholarship recipients.
